Confidentiality Statement

This clinic is bound by a professional code of conduct.  Your privacy is paramount and as such all information will be protected as confidential. This privacy policy has been set out to inform you of the way in which any personal information that you relate to me during a consultation may be used.  I  protect any information given using appropriate physical, electronic and managerial procedures to prevent unauthorised access and ensure that the information is used correctly.

Personal and health related details:  Any information that you disclose as a client will remain strictly confidential, unless you specifically request otherwise, or it is otherwise required by the laws governing the State of Queensland or Australian Federal law. You may obtain access to the information contained in your case file.

Please read the following and sign to indicate your understanding and declaration of consent:

·         I understand that treatment conducted at Scribbly Gum Wellness Clinic does not take the place of medical treatment where needed and it is my responsibility to inform the practitioner of any illnesses, injuries, medications, medical conditions or procedures and any other information regarding my health.

·         I understand that whilst I am not obliged to provide any information, failure to provide full health details requested during consultations may compromise the quality of treatment provided.

·         I understand that a 50% cancellation or no show fee may apply and that 24hrs notice of cancellation is required.

·         I consent to taking Naturopathy/ Bioresonance Therapy / Homeopathy / Kinesiology or other complementary therapy as the preferred treatment of choice.

·         I agree that Angela Stafford carries no liability expressed or implied for the success or failure of any treatment.

·         I understand that Angela Stafford  is not a medical doctor.

·         I understand that no medical diagnosis will be made by Angela Stafford.

·         I understand that any treatment or advice I receive from Angela Stafford  in no way to takes the place of a medical doctor’s care.

·         I agree to inform Angela Stafford about any new or change in medications I am taking.

·         I agree to inform Angela Stafford  if I am pregnant or intend to become pregnant
